Part I. All Junior and General Clubs: Follow these directions to use the following form to report all department, program, and committee activity to the GFWC West Virginia STATE CHAIRMEN.

1. Click on the red GFWC West Virginia Reporting Form 2006-2008 link below. You will need the Adobe  Reader program installed on your computer to open this form. (If you need  this program, it is available on the Documents/Files page of this web site.)

2. This form is a template so you now have two options; you may print this form and complete it by hand or you may use your computer keyboard to complete the form and then save and/or print.

3. Adhere to all the 2006-2008 GFWC Reporting Guidelines in the GFWC Club Manual. (These guidelines can also be found at the red GFWC Reporting Form link listed in Part II below.) You should also refer to the GFWC West Virginia Reporting Books. Additionally, follow all instructions printed on the form.

3. Adhere to all the 2006-2008 GFWC Reporting Guidelines in the GFWC Club Manual. (These guidelines can also be found at the red GFWC Reporting Form link listed below.)  Additionally, follow all instructions printed on the form.

*Please note: the GFWC West Virginia Reporting Form has the lighthouse logo on it and should be used to report activity to our state chairmen only. The GFWC Reporting Form does not have the lighthouse logo on it and should be use to report club activity to the GFWC Headquarters.
